The Salvation Army has partnered with Lutheran Care, Baptist Care, Mission Australia, Sonder, Hutt St Centre and Aboriginal Family Support Services under the banner of the Toward Home Alliance.

Toward Home provides homelessness supports to the Adelaide inner city, Southern metropolitan region and parts of the Adelaide Hills.

Assertive outreach

Our experienced Resolve and Wellbeing (Sonder) teams provide Assertive Outreach to the Adelaide inner city are twice a day, 7 days a week. The Outreach teams tour the streets of the inner city and surrounding parklands to meet with individuals who are street sleeping and connect with them to assess their needs for housing or other specialist supports. Referrals for homelessness case management or other wellbeing/identified support needs are made as appropriate.

If you are aware of someone street sleeping in the Adelaide inner city area, please visit https://www.streetconnect.sa.gov.au/home/launch to make a referral to our Assertive Outreach team.

The Resolve team work closely with all councils in the Toward Home Alliance area and will provide other targeted Assertive outreach outside of the Adelaide inner city as required.

Case Management Support

We recognise that for many people who have experienced long term homelessness there are multiple barriers to accessing housing and challenges in maintaining housing. Participants in the Resolve program receive one-on-one case management support through advocacy and referrals to address housing needs as well as other concerns such as physical and mental health concerns, financial difficulty, long term substance misuses, family/relationship breakdown and legal issues.
